NTSB's preliminary report on fatal Tesla Model X crash in Mountain View in March, 2018

This is the actual NTSB preliminary report.  It includes a second-by-second account of events leading up to the crash.  The driver did not have his hands on the wheel, lane-keeping steered the car out of the lane, and the car began accelerating, failing to detect the barrier.
